Minor - Permission to Travel
Permission to travel from the parent(s)/guardian(s) not present during submission of visa application, legalized/attested/apostilled by MEA/MFA except in cases of a parent having sole custody or guardianship of the minor in which case a court order or other proof of sole custody or guardianship must be provided legalized/attested/apostilled by MEA/MFA. At least one of the parents/legal guardians shall be present for the submission of the visa application

Important Notes

For Indian applicants: GST registration number must be included for companies based in India when applying as company owner or self-employed

For Indian applicants: All documents confirming family ties (birth certificates) must be legalized/attested/apostilled by MEA/MFA

For dependent spouses: Certificate of marriage must be in Polish or English and legalized/attested/apostilled by MEA/MFA

Travel Medical Insurance must have minimum coverage of 30,000 euro

Handwritten insurance certificates will not be accepted

Applicant's passport remains at the Embassy/Consulate General of the Republic of Poland while the visa application is being processed

Applications submitted less than 45 days before intended travel date are accepted on applicant's own risk

The Embassy/Consulate General reserves the right to request additional information/documentation and conduct interviews if deemed necessary

All required documents must be submitted in Polish or English version

At least one parent/legal guardian must be present for submission if applicant is a minor
